In the interest of full disclosure, regardless of HIPPA privacy guidelines, when this receipt is submitted to an employment EAP or health insurance plan, your employer could inadvertently acquire knowledge of your mental health diagnosis and treatment. Your insurance company might electronically outsource their processing to another country , making your health records and personal identity information vulnerable to internet hackers and identity thieves. Establishing a mental health diagnosis may work against you in the event of some insurance claims, such as for car accidents. Your privacy is only assured if you don’t file for reimbursement.

Cancellation Policy
If you do not show up for your scheduled appointment, and you have not notified me at least 24 hours in advance, you will be required to pay the full cost of the treatment as booked.
